Community Care Team

We have a Community Care Team who are based at the Fountains Health in Chester City Centre. The Community care team is made up of District Nurses, Occupational Therapists, Physiotherapists, Community Matrons, and a Social Worker. District Nurses

The District Nursing team provide home nursing care to patients who are unable to come to the surgery through ill health. Arrangements can be made by the surgery for a district nurse visit. The District Nursing team can be contacted on 01244 393413. Please be aware that you will need a referral from the surgery for this service.

Health Visitor

Our health visitor can give advice and help on health care, particularly for expectant mothers, babies, children, the elderly and disabled.

Appointments can be made with the Health Visitor by contacting the team on 01244 348022.

The Starting Well service

Children’s centres provide a wide range of services for prospective parents, children and their families. Starting Well aim to improve the lives of children and their parents by bringing together early childhood services in the heart of the community. Midwife and Maternity Medical Services

The community midwife shares your antenatal care with the doctor. The midwives can be contacted on 01244 365166, you can also book in with the midwife via this website Maternity Services Self-Referral
