Annual Reviews

All patients with a chronic disease (see below table) will be reviewed annually by our Practice Nurses. You will be invited for your review by letter, around your birthday. If you do not receive a letter and have a chronic disease, please contact Patient Services.

It is important that you attend the surgery for your annual review as this allows our clinical staff to complete and check your bloods, review your blood pressure, document and review your BMI as well as looking at general vital signs. In some instances, it is important to have your kidney function checked annually as well as having your medication's reviewed to ensure that they are working correctly. In doing so the nurse will also confirm if your condition is well controlled.

For asthmatic patient's it is important to have your inhaler technique checked by a qualified and trained nurse, even if you are showing no symptoms - By doing so, you can hopefully keep your asthma under control all by just attending a 20 minute appointment, once a year.

Your annual review also provides you with a chance to ask questions around your condition and helps you to be involved in your own care. You can then, in partnership with one of our clinicians, plan your care and learn how to manage your condition effectively. 

Free NHS Health Checks

We offer full Health Checks to all patients aged 40 - 74. We will be systematically asking all our patients to come for one of these over a 5-year rolling period. However, if you would like to have a health check before your recall, please ask our Patient Services Team.

Routine Health Checks

Patients between 16 and 75 years old who have not been seen by a doctor for three years may request a general consultation with a doctor. The doctor shall make such inquiries and undertake such examinations as appear to be appropriate in all the circumstances.

Patients aged over 75 years who have not been seen by a doctor for twelve months, may request a general consultation with a doctor.
